The care navigation library

Explore the work around a care decision: finding options, comparing programs, preparing appointments, involving supporters and tracking follow-through.

A good care search has several connected stages. This library groups those stages so readers can find the question they are trying to answer without starting the whole process again.

Finding a starting point

Identify the kind of conversation you need and the routes available to reach it. NIMH lists several ways to find a professional, including a primary care referral, an insurer’s network, government resources and professional directories. Record where each option came from and when you last checked it.

Comparing providers and programs

Ask consistent questions about experience, methods, staff credentials, practical supports, cost and family participation. Our guide organizes those answers into a comparison sheet. A blank entry means the question still needs an answer; it is not evidence for or against the provider.

Preparing the first appointment

Gather the questions you want answered, the person’s goals, a concise description of the concern and relevant administrative details. Ask the provider what records they want in advance and how they prefer to receive them.

Coordinating the people involved

A family member, treating professional, coordinator and payer may have different responsibilities. Identify who owns each next step and what information that person needs. Include the person receiving care in communication decisions.

Following through

Track appointment dates, confirmed costs, outstanding documents and the next conversation. A decision is easier to act on when someone has accepted responsibility for the immediate task.

Start with our Practical Guides for comparison questions, then use Resources to turn the answers into an appointment brief and next-step log.
